Volunteer Forms & Resources

• Payment Authorization / Request for Reimbursement Form - Chairpersons and committee members, please note: lost reimbursement checks will not be reissued. 

• TUSD Volunteer Form - Must be completed at the start of every school year

• Facilities Use - Chairpersons must follow District policies for use of school facilities

• Vendor Insurance Requirements - Includes instructions and blank/sample forms

• Event Planning Worksheet - For chairpersons to use for planning events/programs

• Committee Report - For chairpersons to complete and submit to the board after event/program completion

• Record Your Volunteer Hours - Be sure to register in Bugle Volunteers to manage your volunteer hours. Volunteer hours are collected and reported in order to maintain Sycamore Magnet Academy's PTA federal tax exemption status
